What is Free Flap Surgery?
Free Flap Surgery, also known as Free Tissue Transfer, is an advanced reconstructive microsurgery technique. It involves transferring skin, muscle, fat, or bone—along with its blood vessels—from one part of the body to another. It is especially useful in treating deep burns, complex wounds, and areas needing both functional and cosmetic repair.
What causes it:
- Extensive burn injuries damaging deep layers of skin and underlying tissue
- Open wounds with exposed bone, tendons, or organs
- Failed skin grafts or previous surgeries that didn’t heal properly
- Severe scarring or contractures limiting movement and appearance

- Deep, non-healing wounds or ulcers that expose bone or tendons
- Severe tissue loss due to trauma, cancer removal, or infection
- Visible deformities or functional impairment in affected areas

Complete Process
Evaluation & Planning
We assess the burn site, tissue condition, and overall health of the patient. Then we determine the donor site (thigh, back, forearm, abdomen, etc.) and plan the surgical approach using imaging and blood vessel mapping.
Tissue Harvesting
A section of tissue (skin, fat, muscle, or bone) is carefully removed from the donor site along with its artery and vein. This tissue is selected based on what’s best suited for the defect (e.g., muscle for strength, skin for coverage).
Microsurgical Transfer & Reconnection
The harvested flap is transplanted to the burn site. Using microsurgery, we meticulously reconnect the blood vessels from the flap to the recipient site to restore blood circulation and promote healing.
Monitoring & Recovery
After surgery, we monitor the flap closely to ensure it stays healthy and properly perfused. Healing takes several weeks, and physical therapy is introduced gradually to regain movement and strength.
