What is an AV Fistula?
An Arteriovenous (AV) Fistula is a surgical connection created between an artery and a vein, usually in the arm, to provide reliable access for hemodialysis in patients with kidney failure. It allows high blood flow, enabling effective dialysis treatment, and is considered the safest and most durable form of vascular access.
What causes it:
- Chronic Kidney Disease (CKD): Patients progressing to kidney failure require hemodialysis, for which a vascular access is needed.
- Poor Dialysis Access from Other Methods: When temporary catheters or grafts fail or cause complications.
- Anticipating Long-Term Dialysis Needs: Planned early in patients with declining kidney function to ensure ready access when dialysis is needed.
Symptoms:
- Decreased kidney function requiring regular hemodialysis
- Inadequate or problematic existing dialysis access
- Frequent catheter-related infections or clotting issues
- Swelling or pain in the arm from existing access complications
- Need for a durable, low-maintenance access point for long-term treatment
Complete Process
Preoperative Evaluation
Our team begins with a detailed vascular assessment using Doppler ultrasound to locate suitable arteries and veins, usually in your non-dominant arm. This ensures the best possible location for creating a strong, durable fistula.
Fistula Creation Surgery
Once the site is marked, the fistula is surgically created under local anesthesia. Our surgeon connects an artery and a nearby vein, allowing blood to flow directly from artery to vein, enlarging and strengthening it over time.
Maturation Phase
After surgery, the fistula needs 4 to 8 weeks to mature. This period allows the vein to become thick enough to handle the dialysis needles and high blood flow needed during treatment.

FAQ
Frequently Asked Questions
An Arteriovenous (AV) Fistula is a surgical connection made between an artery and a vein, typically in the arm, to create a reliable access point for hemodialysis in patients with kidney failure.
An AV fistula provides:
- Higher blood flow for efficient dialysis
- Lower risk of infections and clotting
- A durable, long-lasting access option for long-term treatment
The procedure is done under local or general anesthesia, so you won’t feel pain during surgery. Mild discomfort or swelling is common after the procedure but typically resolves within a few days.
It usually takes 6 to 8 weeks for an AV fistula to mature and be ready for hemodialysis. Your doctor will check its function before first use.
Most AV fistulas are made in the forearm or upper arm, depending on the quality of the blood vessels and the patient’s overall health.
- Keep the area clean and dry
- Avoid lifting heavy weights with the affected arm
- Don’t sleep on the arm with the fistula
- Report any signs of swelling, redness, or pain to your doctor immediately
